Sarah Rich, RN, CCM

Sarah Rich is a healthcare leader, certified case manager, and passionate advocate dedicated to improving outcomes for individuals, families, and communities. As the PHSO Ambulatory Care Management Clinical Program Manager at MercyOne, she leads clinical programs focused on care coordination, population health, quality improvement, and patient-centered care. With extensive experience in healthcare leadership and case management, Sarah is committed to advancing innovative solutions that support patients across the continuum of care.

Sarah serves on the Case Management Society of America (CMSA) Chicago Board of Directors and is leading the growth and development of the CMSA Iowa Extension. In this role, she is focused on expanding professional networking, education, mentorship, and leadership opportunities for case management professionals throughout Iowa. Her vision is to strengthen the profession by fostering collaboration, elevating best practices, and advocating for the vital role case managers play in healthcare.

Beyond her professional accomplishments, Sarah is deeply committed to community service and advocacy. She is a founding board member of Fostering Hope for Second Chances, a nonprofit organization dedicated to supporting foster and adoptive families. As an adoptive mother of twin boys from foster care, she brings personal experience and heartfelt passion to her work supporting vulnerable children and families.

Sarah is also an active advocate within the Down syndrome community, working to promote inclusion, awareness, education, and opportunities that empower individuals with Down syndrome and their families. Through her advocacy efforts, she seeks to create greater understanding, foster acceptance, and ensure that all individuals have the opportunity to thrive and reach their full potential.

Known for her collaborative leadership style and servant-hearted approach, Sarah is driven by a belief that meaningful change happens when healthcare, community organizations, and families work together. Whether advancing the field of case management, supporting foster and adoptive families, or championing disability inclusion, she remains committed to making a lasting impact through compassion, advocacy, and leadership.
